We are committed to giving you the best  surgical
care possible.
In order for us to gather your information and prepare your medical
record, it is important that you bring your insurance card(s), insurance
referrals, related x-rays, lab reports and medical records
relevant to the visit, as well as a list of any current medications
you are taking. If this is the first time that you have
visited our new office, we will be filling out your patient
information into our computer system for the first time.
To help us with your initial medical information or
billing information, you may download a copy of our medical and
patient information sheets to print and fill out prior to your
office visit. These can then be faxed to us, or you may bring the
filled-out forms with you on your next visit. The information in
these forms will need to be updated whenever you change insurance
companies or have new medical information that needs to be entered
into your medical record.

To download, "right click" with your mouse on each
document and a menu will appear that allows you to "save target file
as" and you can save it in any folder or directory on your computer.
Then you can open it with Adobe Acrobat Reader and print it.
You
can now also fill out these forms with your Adobe Acrobat Reader on
your computer and then send the filled out form to our office at the
email address of:
info@wardellortho.com . After you finish filling out
the form, go to the "File" at the top of Acrobat Reader and from the
drop-down menu choose either "Send by email" or "Attach to email",
whichever version of Adobe Acrobat Reader you have. This will bring
up your email program with this document as the attachment. You then
enter the email address of
info@wardellortho.com
into the "TO" address field and send the document.
